SCAGO supports individuals with sickle cell disease regardless of their stage in life. We offer programs to support their transition from one stage of life to another. From adolescence to adulthood to becoming senior citizens as well as provide them with counselling and navigation support. As one aspect of our transition program, SCAGO organizes a yearly Learning for Life educational session to support adolescents with sickle cell disease transitioning to adulthood by bringing experts to support them in the planning process for post-secondary as well as pediatric to adult hospital transitioning. Currently, SCAGO supports the Hospital for Sick Kids, the Toronto General Hospital, the Children Hospital of Eastern Ontario, and the Ottawa Hospitals’ transition program.
